Community HealthChoices (CHC)

How to apply

If you need long-term services and supports, contact the Independent Enrollment Broker (IEB). The IEB schedules an in-person assessment wherever you are (at home, in a hospital or in a nursing facility) to decide whether you need them, helps you complete a Medicaid application for the county assistance office, and talks with you about the CHC plans in your area.
