How to apply
Participant Directed and Managed Services (PDMS)
- How to apply
-
Contact an Options Counselor at your local ServiceLink Aging and Disability Resource Center. The counselor explains the requirements, screens you for Medicaid, and gives you the Medicaid application. Return the completed application to the counselor or your local DHHS District Office. A community nurse then assesses whether you meet the clinical requirements. If you are found eligible, you choose a CFI case manager who plans your services with you. Tell the case manager you want PDMS. The case manager builds it into your care plan and refers you to a Financial Management Services provider.
The PDMS steps are from the state's Provider Notice Manual PDMS/FMS 001.
