Who qualifies
AD Waiver participants who live with and are cared for by someone, when they cannot pay for respite and there is a need for the service. Respite can relieve your ongoing, continuous caregiver, your spouse, your parent when you are an adult, or your adult sibling. It pays someone to come into the home so the primary caregiver gets a temporary break.
You can only receive respite from one funding source. If you are not on a waiver, Lifespan Respite may help.
Unpaid family caregivers who need a temporary break from giving ongoing, full-time care to a person of any age with special needs, such as a physical disability, chronic illness, or Alzheimer's disease or dementia. The person applying must have a family caregiver living in the same home. Eligibility is based on income, and expenses directly related to the disability can usually be subtracted from counted income.
It is not for people who receive respite services from another government program.
Up to $125 per eligible client per month for planned respite, plus up to $2,000 per eligibility year for Exceptional Circumstances, including crisis respite.
Exceptional Circumstances funding needs its own application.
Apply for the waiver online with iServe, or fill out application DD-10, sign it and send it by mail to HCBS Waiver Eligibility, PO Box 98947, Lincoln NE 68509-8947, by email, or by fax to (402) 328-6257. A waiver application is separate from a Medicaid application, and you must have Medicaid. Once you have Medicaid, the state calls to schedule a Level of Care assessment. Respite is one of the services on the AD Waiver, and your Service Coordinator helps build your plan.
Send the Lifespan Respite Services Program application, with any supporting documents, by email to dhhs.respite@nebraska.gov or by mail to DHHS, Division of Children & Family Services, Attn: Lifespan Respite, PO Box 98933, Lincoln, NE 68509-8933. A Respite Network Coordinator in your area can help with the application.
